Hospice Care Coordinator-Home Care

Visiting Nurse is seeking a Hospice Care Coordinator to coordinate and supervise hospice services to individuals and families in Allen County and surrounding region. This full-time benefited position makes supervisory visits with hospice home care staff to ensure requirements are met.

Responsible for staff assignment and scheduling for patients in designated service area. Manages productivity and overtime for assigned staff.

Coordinates Interdisciplinary Team (IDT) home care weekly meetings to assure the provision of quality services to patients in hospice programs in the residential settings.

Supervises preparation of interdisciplinary care plans, the implementation of these plans, and the clinical review and recertification of plan of treatment.

Works with the Home Health Coordinator to ensure care of patients in Hospice and Home Health are managed.

Assures that documentation in the patient record is accurate, objective, comprehensive and verified in required time period.

Works with admission department to achieve purpose of continuity of care and manage requests for patient services according to the admission and discharge policies of Visiting Nurse.

Performs visits, takes referrals and/or admits hospice and palliative care patients, if necessary, to supplement staffing needs.

Interviews, hires, orients and supervises hospice home care staff, and supervises assigned Skilled Nurses and Home Health Aides, with other duties as assigned.

Social Worker PRN Home Care

QUALIFICATIONS:
Masters’ degree in Social Work required from an accredited school of social work for palliative care program. Bachelor’s degree in Social Work required from an accredited school of Social Work for hospice program. Minimum of one (1) year experience in provision of medical social work services, or has a Baccalaureate degree from a school of social work accredited by the Council on Social Work and is employed by hospice prior to December 2, 2008 and is not required to be supervised by an MSW.

Concrete services include meals, finances, benefits, entitlements, funeral planning, volunteer services.

Visiting Nurse is looking for an organized and driven Staff Accountant to join our growing team at Visiting Nurse. Our finance department keeps Visiting Nurse running smoothly so we can deliver the best care to our patients. The Staff Accountant position will work closely with our Finance Director and other operations personnel and handle day-to-day bookkeeping.
