Visiting Nurse is seeking a full-time Social Worker. This person will provide all social service intervention for patients and their families who are enrolled in an agency program as appropriate, provide education/consultation to all staff and volunteers to aid in grief and loss issues, provide education in the community on agency programs and end-of-life issues.
Masters’ degree in Social Work required from an accredited school of social work for palliative care program. Bachelor’s degree in Social Work required from an accredited school of Social Work for hospice program. Minimum of one (1) year experience in provision of medical social work services, or has a Baccalaureate degree from a school of social work accredited by the Council on Social Work and is employed by hospice prior to December 2, 2008 and is not required to be supervised by an MSW.
Must have knowledge and ability to determine appropriate social service roles required to assist patient/family in maximizing the independent functioning that was impaired by physical illness of the patient.
Concrete services include meals, finances, benefits, entitlements, funeral planning, volunteer services.
Counseling services focus on addressing fear, powerlessness, anger, apathy, hopelessness, and facilitate communication and understanding among family members. Must hold Indiana license or temporary licensure and actively working on permanent license.
Ability to function independently and to work with medical professionals in the community.
Ability to establish rapport with a wide variety of individuals.
Must be able to safely perform the essential functions of the job without posing a direct threat to the health and/or safety of self or others in the workplace.
Must hold CPR certification or obtain within the orientation period.
