Visiting Nurse has been providing comprehensive support for those who are seriously ill in Northeast Indiana since 1888. Visiting Nurse has several programs to meet the needs of patients and families including home health, palliative care, hospice services and grief support.
Golden Years Homestead is a premier continuing care retirement community that has served the greater Fort Wayne area since 1971. Golden Years has options to support the needs of residents and families including independent living, assisted living, rehabilitation and skilled nursing facilities to support health and memory care needs.
Visiting Nurse and Golden Years are non-profit organizations partnering together with an emphasis on doing what is best for residents, patients and their families.
A new option for patients in need
Visiting Nurse has opened a 5-bed unit at Golden Years Homestead, located on the Northeast side of Fort Wayne at 3136 Goeglein Road, staffed around the clock by Visiting Nurse staff providing high-quality and patient-focused hospice care. Patients are eligible for care at Visiting Nurse’s unit at Golden Years after being transferred from an acute care setting or hospital if they have a prognosis of 2 weeks or less, do not have a caregiver or are in a caregiver crisis situation. Patients will have symptoms managed and controlled prior to admission to Visiting Nurse’s unit, where they will receive a routine level of hospice care.
